Gothika is the story of a psychiatrist, Dr. Miranda Grey, who works at an asylum and has a car accident after trying to avoid a girl on a road during a stormy night, while driving back home. She rushes to try to help the girl, who turns out to be a ghost. The girl, in turn, gets possession of Miranda's body by burning Miranda after Miranda extended her hand to the girl.
Miranda next wakes up in the same asylum that she used to work for, but she wound up there as a patient instead. Drugged and confused, she barely remembers what happened after the car accident. The ghost uses Miranda's body to carry out messages, most noticeably the "not alone" notes that she cut into Miranda's arm, which, in turn, led other doctors to believe Miranda' was suicidal.
The ghost had used Miranda's body to murder Miranda's husband, and all the evidence pointed towards Miranda as the killer.
Eventually, Miranda escaped the asylum, having recognized the girl as a ghost and recovered some of her memory. She then discovered that the girl, as well as one of Miranda's former patients and many other teenaged victims had been raped by her husband and by the local sheriff, who was still alive.
Having also discovered that the girl, who died by committing suicide after being raped, was also the daughter of her former chief at the asylum, (and that the girl's father also experienced occasional visions of his daughter), Miranda is then attacked by the local sheriff, who tries to rape and murder her. However, Miranda kills the sheriff in an act of self defense.
Being proven not to be insane or guilty of any crimes, Miranda and her friend-patient are let go free.
